    Type Alias ConceptDocument

    One document a concept's matchers recognise. documentedAt is null for a chart the EMR gave no date, and isReady is false while it is still ingesting — both are returned rather than filtered out, because a document the evaluator cannot see is the difference between a confident zero and an honest "not yet".

    PHI: title is a document title. It belongs in rule_evaluation.result, never in a log.
