Where signatures came from. extractor is extractEmrSignatures reading a structured signature
field, and unavailable is a document type whose raw_document has no such field to read.
The distinction is the whole point: an empty array under extractor means we looked and found
none, while an empty array under unavailable means nothing looked at all. Six of the document
types are covered, so unavailable is the common case — reading it as "unsigned" is how a signed
consent form becomes a false compliance answer.
